Log in to the Electronic Declaration System.
In the electronic declaration system, in the "Declaration" section, select "Fill in the form".
Complete form FR0512. Entering the "Form code:" field will make it easier to find the required form.
Enter MRUSA legal entity code in the recipient search field: 191991519.

(this is the code of "Mykolas Romeris University Student Representation" as a beneficiary).
In the field "Amount of tax share" enter the amount of support you want in percentage. It can be up to 1.2 percent.
After filling in all the data, click "Form a request".
